On page two (2) of the Passport Application Form, there is a rectangular signature box at the top of the page (the middle box).

• Please sign in the MIDDLE of the box.
• The signature MUST NOT touch any of the lines.
• Applicants MUST also sign declaration in Section E.
• Both signatures MUST be IDENTICAL.
• Minors over the age of 12 MUST sign in the box
• The parent/legal guardian submitting the application for a minor MUST complete and sign section C and E of the application form.
