Section G of the completed application form MUST be certified by one of the following:

• Justice of the Peace
• Notary Public
• Attorney-at-law
• Medical Doctor
• Minister of Religion authorised to perform marriages
• Consul/ Honorary Consul (if applicant appears in persons)

The official who certifies the application is also required to certify the reverse side the photograph of the applicant, with the following inscription above his/her signature: “I certify that this is a true photo of (name of applicant)”.  (See Photo Guide attached)
