Jamaican Passport Application Guide

|Documents Required

Along with the completed application form you will be required to submit the following documents:

Adult Passport Information

First Time Adult Passport 

  • Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
  • Two (2) identical photographs
  • A valid Government issued Photo Identification (E.g.: Drivers Licence, Voter Identification)
  • Proof of name change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)

Renewing Adult Passport 

  • Current Passport
  • Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
  • Two (2) identical Photographs
  • Proof of name Change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)

Replacing a Lost, Stolen or Damaged Adult Passport

You will be required to obtain and present a police report for the replacement of your lost/stolen or damaged passport.

  • The official police report
  • Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
  • Two (2) identical Photographs
  • A valid Government issued Photo Identification (Example: Drivers Licence, Voter Identification).
  • Proof of name change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)

Child Passsport Information

  • The mother, father or legal guardian must complete the application for a child less than 18 years of age.
  • The parent/legal guardian submitting the application must complete and sign sections C and E of the application form.
  • Minors over the age of 12 must sign in the signature box in the middle at the top of page 2.

  • The Minor’s Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
  • Two (2) identical Photographs
  • A valid Government issued Photo Identification or passport for the parent/legal guardian (Example: Drivers Licence, Voter Identification).
  • Proof of name change if name has been changed (Deed poll or Court Order)

Renewing a Child Passport

  • The Minor’s Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate
  • Two (2) identical photographs
  • A valid Government issued Photo Identification or passport for the parent/legal guardian (e.g.: Drivers Licence, Voter Identification).
  • Proof of name change if name has been changed (Deed poll or Court Order)
  • Current passport

Replacing a lost/stolen or damaged minor (under 18yrs) passport

You will be required to obtain and present a police report for the replacement of the lost/stolen or damaged passport.

  • The official police report
  • Birth Certificate/Adoption Certificate or Jamaican Citizenship Certificate for the minor
  • Two (2) identical Photographs
  • Proof of name change if name has been changed (Marriage Certificate, Deed poll, Divorce Decree, Court Order)
  • A valid Government issued Photo Identification for the parent/legal guardian (e.g.: Drivers Licence, Voter Identification).
